This questionnaire is anonymous, we will not ask for your name or address.

We have structured the questions into five topics which can be answered in just a few minutes.

You don’t have to answer every question and you don’t have to write comments for each point. But the more you tell us the better!

Please rate us on a scale from 1 to 6, 1 being the BEST, 6 being the WORST. If you do not want to rate a certain issue, simply skip the question.
